2. Fill out the Personal Data Sheet (CSC Form 212, Revised 2017) and get it notarized.
3. Include a Certificate of Employment from your previous job.

5. Make certified true copies of your Transcript of Records (your educational records).
6. If you have a PRC license or Civil Service Eligibility, make a certified true copy of it (if applicable).
