3. License and Board Rating / Certificate of Eligibility (if new applicant)
4. Transcript of Records (TOR) (if new applicant)
5. College Diploma (certified true copy, if new applicant)
6. Certificates of Relevant Training (if applicable)
7. Certificate of Live Birth (original, if new applicant)
8. Marriage Certificate (original, if applicable)
9. NBI Clearance (original)
10. Court Clearances (RTC, MTC, and Prosecutor’s Office where you reside, if new applicant)
11. Individual Performance Commitment Review (IPCR) – last two rating periods (if applicable)

Step 3. Submit Your Application
