- Personal Data Sheet (PDS) – 2 original copies of CS Form No. 212 (Revised 2017) with Work Experience Sheet and recent passport-size picture (taken within 6 months; not computer-generated or photocopied).
- License and Board Rating / Certificate of Eligibility (if new applicant)
- Transcript of Records (TOR) (if new applicant)
- College Diploma (certified true copy, if new applicant)
- Certificates of Relevant Training (if applicable)
- Certificate of Live Birth (original, if new applicant)
- Marriage Certificate (original, if applicable)
- NBI Clearance (original)
- Court Clearances (RTC, MTC, and Prosecutor’s Office where you reside, if new applicant)
- Individual Performance Commitment Review (IPCR) – last two rating periods (if applicable)
