How to Apply
Here’s a step-by-step guide on how to apply for the position:
Step 1: Register Online
- Applicants must register through this link: DOTr-Central Office Online Application.
- Click on it. This is where you begin the application process.
Step 2: Background Check Form
- Next, you’ll need to fill out a Background Check Authorization Form. It’s like giving permission for the DOTr to check your background.
- You can find the form at http://bit.ly/BackgroundCheckAuthorizationForm. After filling it out, save it as a PDF file.
Step 3: Upload the Form
- Now, go back to the DOTr-Central Office Online Application. You’ll see an option to upload files. Click on it, and then upload the PDF file you just filled out.
Step 4: Write an Application Letter
- Write a letter addressed to the Human Resource Management Division Chief. In this letter, kindly indicate which position you want to apply for and mention any other positions you might be interested in.

Step 5: Fill Out the Personal Data Sheet
- Download and complete the Personal Data Sheet (CS Form No. 212, Revised 2017)
- Don’t forget to include a recent passport-sized photo.
Step 6: Eligibility, Transcript, and Diploma
- Scanned copy of Authenticated CSC Eligibility/BAR or Board Rating/License (if applicable);
- Scanned copies of Authenticated Transcript of Records and Diploma;
- Scanned copy of Certification as to units earned in Post Graduate Studies (if not yet graduated in Masteral or Doctoral studies);
- Scanned copies of Certificates of Relevant Training/Seminars attended;
- Scanned copies of Service Records/Certificate of Employment;
- Scanned copy of the Performance rating in the last rating period (if applicable);
Step 7: List of Relatives
- If any of your relatives work in the DOTr, list their names.
Step 8: Other Credentials
- If you have any awards, have been a resource speaker, worked on innovations, authored materials, or participated in programs and projects, provide details and evidence of these in your application.
Important Notes:
- Make sure you provide all the required documents. Incomplete applications won’t be considered.
- Your application will be valid for one year.
- If you’re shortlisted, the DOTr will contact you and ask to see your original documents for verification.
- The DOTr is very serious about confidentiality and treating everyone fairly, regardless of age, gender, disability, religion, or other factors.
Applications with incomplete documents shall not be entertained.
Applications will be considered valid for a period of 1 year. Only applicants who are shortlisted will be contacted.
Shortlisted applicants will be required to show original copies of their documents for verification. All information submitted by applicants shall be subject to verification and/or background checks.
