- Affidavit of Informed Consent, Waiver, and Undertaking:

- Individual Performance Commitment Review (IPCR) or Equivalent (for applicants with work experience):
- Submit a scanned copy of the certified true copy of the completely signed IPCR or its equivalent for the last rating period.
