Document Requirements

Applicants are required to submit the following:

  1. Application Letter:
    • Send a scanned copy of the signed application letter.
    • Mention the job title, salary grade (SG) level, and job location.
    • You can apply for up to two positions in one posting period.
    • Address the letter to:
      • Claire Dennis S. Mapa, PhD
      • Undersecretary
      • National Statistician and Civil Registrar General
  2. Personal Data Sheet (PDS):
    • Include a scanned copy of your completed Personal Data Sheet (PDS) with a recent ID picture (taken within the last three months).
    • Must be signed and sworn before an authorized officer before the deadline.
    • Use CSC Form No. 212, Revised 2017.
  3. Work Experience Sheet (if applicable):
    • Include a scanned copy of the signed Work Experience Sheet (CSC Form No. 212 – as Attachment to PDS).
  4. Proof of Eligibility (if available):
    • Provide scanned copies of proof such as CSC, RA 1080, PD 907, etc.
  5. Transcript of Records and Diploma:
    • Include scanned copies of both documents.
  6. Certificates of Training (if applicable):
    • Provide scanned copies of relevant training certificates.
  7. Certificates of Work Experience (if applicable):
    • Provide a scanned copy of Certificates of Work Experience relevant to the position (for applicants with work experience).

    • Missing important entries on the PDS, such as the applicant’s signature on each page or the authorized signature on page 4.
    • Incomplete attachments or missing information/signatures in the attachments.
  5. The HR board or hiring unit will schedule interviews for pre-qualified applicants. Online interviews are possible, and any changes will be notified one day in advance via email or phone.
  6. Selected applicants must submit a Certification under Oath Against Nepotism (Attachment 1).
