For Lawyer Positions:
- Application Letter (addressed to the LRA Administrator)
- Personal Data Sheet (PDS) with Work Experience Sheet and recent photo (CS Form No. 212, Revised 2025)
- Certificate of Admission to the Bar
- Certificate of Bar Rating
- Transcript of Records (TOR)
- Hard copy or electronic copy.
- Certificates of Relevant Trainings/Seminars
- Photocopy of certificates relevant to the position applied for (preferably MCLE Compliance).
- NBI Clearance
- Updated NBI Clearance (for new/transfer applicants).
- For Promotion/Transfer (if applicable):
- Hard copy or electronic copy of Performance Rating in the last rating period.

- Clearances (valid for 6 months):
- NBI Clearance
- CSC Clearance
- Sandiganbayan Clearance
- Ombudsman Clearance
- If not available: Affidavit of Undertaking
- For applicants with pending cases (if applicable):
- Signed case brief/executive summary
- Recommendation letter from Head of Agency
- Copies of complaint/affidavit with attachments
- Counter affidavit and supporting documents
- Rejoinder and related documents
- Other relevant decisions (if any)
- Certification from recommender stating that you meet all qualifications and have no disqualifications
- For Promotion/Transfer (if applicable):
- Certified true copy of Performance Evaluation (at least VS rating for the last two semesters)
