For Lawyer Positions:

1. Application Letter (addressed to the LRA Administrator)
2. Personal Data Sheet (PDS) with Work Experience Sheet and recent photo (CS Form No. 212, Revised 2025)
3. Certificate of Admission to the Bar
4. Certificate of Bar Rating
5. Transcript of Records (TOR)
   • Hard copy or electronic copy.
6. Certificates of Relevant Trainings/Seminars
   • Photocopy of certificates relevant to the position applied for (preferably MCLE Compliance).
7. NBI Clearance
   • Updated NBI Clearance (for new/transfer applicants).
8. For Promotion/Transfer (if applicable):
   • Hard copy or electronic copy of Performance Rating in the last rating period.

For Presidential Appointee Positions:

1. Application Letter (addressed to the LRA Administrator)
2. Personal Data Sheet (PDS) with Work Experience Sheet and recent photo (CS Form No. 212, Revised 2025)

5. Clearances (valid for 6 months):
   • NBI Clearance
   • CSC Clearance
   • Sandiganbayan Clearance
   • Ombudsman Clearance
   • If not available: Affidavit of Undertaking
6. For applicants with pending cases (if applicable):
   • Signed case brief/executive summary
   • Recommendation letter from Head of Agency
   • Copies of complaint/affidavit with attachments
   • Counter affidavit and supporting documents
   • Rejoinder and related documents
   • Other relevant decisions (if any)
7. Certification from recommender stating that you meet all qualifications and have no disqualifications
8. For Promotion/Transfer (if applicable):
   • Certified true copy of Performance Evaluation (at least VS rating for the last two semesters)
