Applicants are required to submit the following documents in person or via courier/email:
- Application Letter indicating the Position Title and Item Number.
- Notarized Personal Data Sheet (PDS) with a recent passport-size picture (CS Form No. 212, Revised 2017) dated within the publication period (March 20–30, 2025.
- Work Experience Sheet dated within the publication period.
- Performance Rating / Performance Evaluation (IPCR/DPCR) for the last rating period (July to December 2024).
- Certificate of Eligibility / Certification of Bar or Board Examination Results, Rating, and License Card.
- Appropriate License required for the position (e.g., Professional Driver’s License) as per CSC MC No. 11, s. 1996, amended by CSC MC No. 13, s. 2013 (Category I, II, III, and IV).
- Authenticated Transcript of Records and Diploma.
- Recent CSC and Ombudsman Clearance for No Pending Criminal and Administrative Cases (for SG-24 positions).
- Certificate of Previous Employment / Service Record (if applicable).
- Certificate of Trainings and Seminars Attended.
- Certificate of Awards and Commendations received and issued by the LTO PRAISE Committee, CSC, other NGAs, LGUs, GOCCs, and NGOs (if any).
- Other Credentials.
