- Work Experience Sheet – dated within the publication period (Sept. 25 – Oct. 05, 2025).
- Performance Rating / IPCR / DPCR – covering the last rating period (Jan. 01 – Jun. 30, 2025).
- Certificate of Eligibility / Bar or Board Exam Results – including rating and license card.
- Appropriate License – if required for the position (e.g., Professional Driver’s License, CSC MC No. 11, s. 1996, as amended).
- Transcript of Records and Diploma – authenticated copy.
- CSC and Ombudsman Clearance of No Pending Case – for SG-24 positions only.
- Certificate of Previous Employment / Service Record – if applicable.
- Certificates of Trainings and Seminars Attended.
- Certificates of Awards and Commendations – received from LTO PRAISE Committee, CSC, or other NGAs, LGUs, GOCCs, and NGOs (if any).
