Step 1: Prepare Your Application Letter

Your application letter must clearly include the following details:

  • Position Title
  • Name of the Department
  • Item Number
  • Salary Grade of the position

Important Notes:

  • If you are applying for more than one vacant position, you must submit a separate set of application documents for each position.
  • Failure to indicate the item number applied for will result in disqualification from the assessment.

Step 2: Complete and Notarize Your Personal Data Sheet (PDS)

Prepare the following:

  • Notarized and Fully Accomplished Personal Data Sheet (PDS)
    • Use CS Form No. 212, Revised 2025
    • With recent passport-sized picture
    • With your name and signature
  • Attach the Work Experience Sheet

Step 3: Gather Required Supporting Documents

Attach clear photocopies of the following:

  1. Performance Rating in the last rating period (if applicable)
  2. Authenticated Copy of certificate of eligibility, rating, or license
  3. School Credentials, depending on position requirements:
    • TOR and Diploma (for Graduate Studies, Bachelor’s Degree, or 2-year College/Vocational/Trade Course)
    • High School or Elementary Diploma or Certificate from DepEd (for positions requiring basic education)
  4. Certificates of Learning and Development (L&D) interventions or training programs attended

Step 4: Prepare Hard Copies Only

  • All applicants are required to submit a hard copy only of their application documents.
  • You may hand-deliver or send via courier. Late submissions will not be accepted.
  • All documents you submit will become the property of PCSO, and the office may take any action it finds appropriate.
