Do I Need to Notarize My PDS?

The Personal Data Sheet (PDS) must be subscribed and sworn to under oath by the applicant on the last page of the form.

This oath may be administered by the agency’s highest-ranking Human Resource Management Officer (HRMO), any authorized official or officer of the agency, or a Notary Public.

Heads of agencies may also delegate this authority through an official Office Order.
