    • Completed and notarized Personal Data Sheet (PDS) with a recent passport-sized photo
    • Updated Work Experience Sheet (WES)
    • Performance rating from the last rating period (if applicable).
    • Photocopy of your certificate of eligibility, rating, or license.
    • Photocopy of your Transcript of Records and Diploma.
    • Photocopy of relevant training certificates.
    • Service Record (for government employees) or Certificate of Employment.
  • Step 4: Address your Application Letter to:
