Step 1: Prepare Your Documents: 

Gather all the necessary documents listed below:

• Letter of Application
• Fully accomplished and notarized Personal Data Sheet (PDS)
• Fully accomplished Work Experience Sheet (WES)
• Service Record or Employment Certificate
• Photocopy of Transcript of Records and School Diploma
• Photocopy of Certificate of Eligibility / Rating / License
• Photocopy of Certificates of Trainings / Seminars attended
• For Government Transferees: Individual Performance Commitment and Review (IPCR) / Division Performance Commitment and Review (DPCR) from the last rating period
• Performance Appraisal / Evaluation for other applicants, if applicable

Step 2: Complete the Personal Data Sheet (PDS): 

• Fill out the Personal Data Sheet (PDS) completely and accurately. 
• Make sure to include your thumb mark, wet signature, and attach a recent passport-sized ID picture with a handwritten name tag and signature over printed name.
• Remember, computer-generated or photocopied pictures are not allowed.

Step 3: Complete the Work Experience Sheet (WES): 

• Fill out the Work Experience Sheet (WES) completely and sign and date it. 
• Ensure that the date matches the date on your PDS.

Step 4: Notarize Your Documents: 

• Get your Personal Data Sheet (PDS) notarized by a notary public. The notarization date should be before the submission deadline.

Step 5: Submit Your Application:
