- Photocopy of Transcript of Records and School Diploma
- Photocopy of Certificate of Eligibility / Rating / License
- Photocopy of Certificates of Trainings / Seminars attended
- For Government Transferees: Individual Performance Commitment and Review (IPCR) / Division Performance Commitment and Review (DPCR) from the last rating period
- Performance Appraisal / Evaluation for other applicants, if applicable
