  2. Fully and properly accomplished Personal Data Sheet (CS Form No. 212, Revised 2017, with signature on every page, latest passport-sized picture and thumbmark) and Work Experience Sheet;
  3. Certificate of Employment from current and previous employers
  4. Updated Service Record (for applicants with government service)
  5. Memorandum of Agreement or Contract (for applicants with Job Order and/or Contract of Service employment)
  6. Scanned Copy of Diploma (certified by the school registrar)
  7. Scanned Copy of Transcript of Records (TOR) (certified by the school registrar)
  8. Scanned Copy of Valid Professional Regulation Commission (PRC) License or Integrated Bar of the Philippines (IBP) License (if applicable)
  9. Scanned Copy of CSC Certificate of Eligibility/ PRC Ratings/ PRC Certificate of Registration/ BAR Ratings/ Certificate
  10. Scanned Copy of Valid Driver’s License (for Driver applicants only)
  11. Scanned Copy of Training Certificates (relevant to the position applying for) as indicated on the PDS (training certificates obtained during College and Internship will not be counted in the evaluation)
  12. Scanned Copy of Individual Performance Commitment Review (IPCR) Form for the last two (2) consecutive rating periods (for government employees)
